Key Responsibilities:
Patient Care Delivery:
Coordinate and deliver patient and caregiver care with a focus on their individual needs.
Conduct nursing assessments to ensure comprehensive care management.
Develop, implement, evaluate, and update care plans to maintain continuity and meet goals.
Empower patients and caregivers to be as independent as possible, providing education as needed.
Follow established protocols and guidelines to provide the highest standard of hospice care.
Educate patients and families, offering time for questions and ensuring clear communication.
Work closely with the interdisciplinary team (IDG), patients, caregivers, and other healthcare professionals to ensure seamless care.
Interdisciplinary Group (IDG):
Attend and actively participate in team meetings and special care conferences.
Cultivate effective working relationships with team members and refer patients/caregivers to appropriate specialists as needed.
Documentation & Compliance:
Ensure that clinical documentation is accurate, complete, and timely in line with guidelines and regulatory standards.
Professional Accountability:
Maintain up-to-date knowledge, certifications, and licensure.
Demonstrate ongoing professional development, staying current with trends and best practices in healthcare.
Maintain productive and professional relationships with external healthcare providers.
Exhibit strong time management skills and adapt to patient needs in crisis situations.
Additional Duties:
Assist with maintaining a clean and safe environment, following patient room cleaning procedures and ensuring the use of appropriate cleaning supplies and safety measures.
Minimum Qualifications:
A Diploma or Associate's Degree in Nursing (Bachelor's Degree preferred).
A current, active, and unrestricted compact (multistate) Registered Nurse (RN) license.
Current CPR certification (CHPN preferred).
Minimum of 1 year of experience in inpatient care and 2 years of nursing experience for other roles. Experience in caring for patients with life-limiting illnesses is required (hospice, palliative care, geriatrics, oncology, home health, or similar).
Computer proficiency is required.
